Activity Hazard Analysis - APP
The APP requires an AHA for each DFOW on the project. Each AHA requires a full list of tools/equipment that will be used for the activity, as well as an overall activity RAC (determined by the activity with the highest RAC). Use “Add New Equipment” and “Add New High Risk Activity” respectively to add as many items as needed. Make and model number, if possible, are recommended for each tool/equipment item added. If there will be external/third party inspections, click “Yes” and describe the inspections (what is being inspected, by who, etc.). Next, choose the template you want your AHA(s) to use. Templates A, B, and F are the most recent (EM 385 2014 version accepted), but templates C, D, and E are still widely used.
